Walking Home Consent Form
Any child who is in Year 4 or above may have permission to walk home alone. Please complete the form and either email it completed to office@st-marys-oswaldtwistle.lancs.sch.uk or complete and return to school by hand. Forms are also available from the school office.
Holiday Request Form
Please complete a holiday request form if your child is going to be away from school during term time. This ensures we know where your child is and we don't need to phone you or complete a home visit. Please return forms to the school office by hand or email to office@st-marys-oswaldtwistle.lancs.sch.uk
Home-School Agreement
We ask all staff, pupils and parents to sign our home school agreement during their time at Saint Mary's.
Annual Consents
Here is our annual consent; this will cover food tasting, film viewing, local walks, visits to other schools and photographs. It is really important we have this returned to school as quickly as possible so your child can participate in the relevant activities.
School Dog Consent Form
Winnie is our school dog and visits us often. All pupils are required to sign a consent form to work with her. This lasts for the duration the pupil is at school unless you specifically request us to change it:
